Data Security Policy

At Personalized Prescribing Inc. (PPI), we take the privacy and security of your personal and genetic information seriously. Our pharmacogenetic testing analyzes metabolic (e.g., CYP2D6, CYP2C19, CYP2B6) and pharmacodynamic genes through 106 single nucleotide polymorphisms (SNPs) across 58 genes.

Clinical Validity and Disclaimers

We provide medication recommendations based on gene-drug interactions that are clinically validated by leading international pharmacogenetics consortia, such as CPIC and DPWG. In cases where a gene-drug interaction is not yet fully validated, we include the information with clear disclaimers and share it only with your healthcare provider for consideration.

Use of Genetic Information

Your genetic data is used solely to guide personalized medication recommendations. We do not conduct whole genome sequencing or assess disease risk. While current test results do not indicate risk for disease, emerging research could associate certain genetic variants with future clinical findings.

Data Protection Measures

We employ encrypted, firewalled databases and secure systems to protect your data. While no system can be guaranteed 100% secure, we take all reasonable steps to prevent unauthorized access. In compliance with Canada’s Genetic Non-Discrimination Act (2017), we do not share your genetic, health, or personal information with third parties without your explicit consent.

Certification and Accreditation

Personalized Prescribing Inc. (PPI) is proudly certified under ISO 9001 for quality management and ISO 27001 for information security. Our laboratory is also accredited by the College of American Pathologists (CAP), reflecting our commitment to clinical excellence.

PPI undergoes annual audits by these accreditation bodies to ensure that the highest standards of quality, data protection, and ethical practices are consistently upheld in both our patient services and data management systems.

Your Privacy and Confidentiality

  • Your data is anonymized using a unique Sample ID to separate personal identifiers from your DNA sample and results.
  • Your genetic information is stored securely for 25 years in accordance with regulatory standards from CLIA and CAP.
  • DNA samples are retained for 90 days, or until the next internal proficiency test—whichever is longer—to support any required reanalysis.
  • Your identifiable report (name, Sample ID, drug recommendations) is shared only with your treating healthcare provider, as permitted by your consent.
  • Access to your personal data is strictly limited to authorized personnel who require it for operational purposes. Appropriate security measures will be maintained to prevent unauthorized access or disclosure.

Research and Aggregate Use

De-identified data may be used in aggregate for internal research, quality improvement, and scientific publications. For example, to understand how specific genetic variations impact medication response at a population level.

Record Retention

If Personalized Prescribing Inc. (PPI) ceases operations for any reason, all physical patient records will be securely destroyed by shredding. However, servers hosting client data will remain active for one year following the date of closure to allow for necessary access and retrieval. After this one-year period, PPI will instruct the server hosting provider to permanently delete and wipe all data from the servers in compliance with data protection standards.

Security Breach

In the event of a security breach resulting in the accidental or unlawful destruction, loss, alteration, unauthorized disclosure of, or access to personal data, PPI will promptly assess the potential risks to individuals’ rights and freedoms. If deemed necessary, the breach will be reported to the appropriate parties in accordance with applicable data protection laws and policies.

Your Rights

  • You may request the destruction of your genetic data at any time. Upon request, we will delete your data within seven days.
  • You may withdraw your consent to share information at any time by contacting us at: 1-844-943-0210 or info@personalizedprescribing.com
