The test is available to individuals who meet the following criteria:
1) Covered under a Manulife Group Benefits plan.
2) Have one of the following covered conditions:
- a mental health condition, such as depression or anxiety
- chronic pain
- neurological conditions
- attention deficit hyperactivity disorder (ADHD)
3) Are either:
- starting a new medication, or
- taking a medication that isn’t working, or
- taking a medication that’s causing side effects
By clicking “Order Test” you are confirming that you meet these criteria.
Benefits Coverage
The test may be covered for you and your family under your group benefits plan.
To check your coverage from the Manulife Mobile app, sign in to your account. Under Get care, select Medical Health. If you have coverage, the Personalized Medicine tile will display “Covered”.
To check your coverage from the plan member site, sign in to your account. Under Get care or Healthcare services (depending on your access), check for information about Personalized Medicine.
You must have active extended health care coverage for the test to be covered (for example, if you opted out of extended health, the test would not be covered).
If you’re placing an order for a family member who’s covered under your plan, please use their name.
To claim under your benefits plan: submit your claim through the Manulife plan member site and attach your receipt.
