Eligibility Criteria
The test is available to individuals who meet the following criteria:

- Covered under a Manulife Group Benefits plan
- Have a mental health condition (depression/anxiety), chronic pain, neurological condition, or ADHD
- Starting a new medication, or taking a medication that isn't working or causing side effects
Benefits Coverage
The test may be covered for you and your family under your group benefits plan. To check your coverage from the Manulife Mobile app, sign in to your account. Under Get care, select Medical Health. If you have coverage, the Personalized Medicine tile will display 'Covered'.
You must have active extended health care coverage for the test to be covered. If you're placing an order for a family member who's covered under your plan, please use their name. To claim under your benefits plan: submit your claim through the Manulife plan member site and attach your receipt.
