The test is available to individuals who meet the following criteria:
1) Covered under a Manulife Group Benefits plan.
2) Have one of the following covered conditions:
- a mental health condition, such as depression or anxiety
- chronic pain
- neurological conditions
- attention deficit hyperactivity disorder (ADHD)
3) Are either:
- starting a new medication, or
- taking a medication that isn’t working, or
- taking a medication that’s causing side effects

Benefits Coverage
The test may be covered for you and your family under your group benefits plan. To check your Group Benefits account, sign in to your account. Under Healthcare services, check for information about Personalized Medicine. You must have active extended health care coverage for it to be covered (e.g., if you opted out of extended health, the test would not be covered).
If you’re placing an order for a family member that’s covered under your plan, please use their name.
To claim under your benefits plan: Submit through the Manulife plan member site and attach your receipt with your claim.
