By proceeding with this test, you consent to taking a drug compatibility test, providing personal and medical history information, and submitting a DNA sample via Canadian mail.

You authorize genetic analysis at the Personalized Prescribing Laboratory in Richmond Hill, Ontario. Your barcoded genetic data will be archived digitally for 25 years per CLIA and CAP standards.

Only a healthcare professional may interpret and utilize the report to improve your health outcome. You should never adjust medications without consulting your healthcare provider.
